Those who are under 18 years of age need this Registration & Release of liability form signed by a parent prior to working out at Spirit Gymnastics. If your parent or legal gardian are not going to be here when you first arrive you can print out this Registration & Release form and have them fill it out and sign in the 3 places, prior to coming To Spirit.

Coaches Contract:
All coaches are required to fill out a Coaches Contract form. To download the form, right click on the icon and click on "open link." Print out the form. Complete the form and bring it with you, along with a completed Registration & Release of Liability form (located through the link above). Print out form.
